Lyric Transfer of Care Explanation

For patients who live in Colorado and Arizona (or just moved here) looking for a new Lyric provider to take care of them. This means that you can come into the office one time for a complimentary visit and replacement of devices (if needed) to meet us and establish your needs. If you have any information about when your Lyric subscription started, who you were working with, any of your records and/or your "P#" - this will be very helpful for the replacement of your Lyric.
